To sign your child up to receive services at Tiny Notes Pediatric Therapies, please fill out our interest form and return it to us. Once we receive your form, we will provide you with an intake packet to complete and return. We require a referral from the child's Primary Care Manager/Provider and may require prior authorization from your health plan.

At Tiny Notes, the wait time for new patient appointments and evaluations can vary based on our current clientele and staffing levels. Please note that specific time frames for the waiting list are not guaranteed, as they are subject to change based on demand and availability.

We require 24 hour notice to be given for missed therapy appointments, or a cancellation fee may be charged. Please call us at 360-443-2399 to let us know in advance.
